Nexium Granules and Pepcid HELP!
All, I need some help. My son has had several issues since birth. He had a laryngeal cleft. One of the complications with this defect is severe reflux. He has been on the highest dose of Nexium you can give a child for a year and the latest scope still showed damage. They now want us to give him Pepcid on top of the Nexium. Does anyone have experience with this? It seems like a lot of medicine. Please help!

Ryan was born May 20th 2008. 9 lbs 2 oz. Ryan’s magic bullet was 30mg of Prevacid capsules mixed with Caracream from Marci-kids (I am forever indebted to the team at Marci-kids), Claratin for the mucos, and Mirlax for the constipation. Within 2 months of this combo he jumped from the 6% to the 50% for weight and continues to climb. Be strong, listen to your instincts. A few parents do PPI's and H2 Blockers both. Hopefully one of them will chime in. My question to you is "What other PPIs have you tried?" Different kids respond to diffent PPIs. My son did not turn around until I started adding Caracream to his PPI. The turn around was drastic. We were days from a feeding tube and once I added the Caracream to the mix, he did a complete 180.

Yes. Ryan was that low in intake. Worse on some days. You need to watch for dehydration and take her to the ER for fluids. We were there every 6 weeks until we found the right meds that worked for him. We were days from a feeding tube. Stay on top of you doctors. Insist on a PPI. Feeding aversions are hard to over come once they set in. Find a different doctor if you need to.

What is the dose you are currently giving?

Is it a solutab, are you getting it compounded?

All sorts of things can play into whether or not the med will be efective.

He is taking 20 mg Nexium Granules right now (a packet of powder). The pepcid they want him to take is just the chewable type. What do you think?

I would say that if you are at that dose and still seeing damage, then NO the dose is not high enough.

for maintenance, my son takes 15mg of Prevacid once a day. So your dose doesn't seem high to me at all. I know kids with doses WAY higher.

But, you always need to do what the DR says. Have they told you why they won't go any higher?

What about adding in a med called Carafate? It's by RX only and it's basically super mylanta to allow the esophagus to heal from damage. It coats it so it doesn't get anymore damage and should allow things to heal.

It can be tricky ti time this med, but it can work well.
