Mom to two refluxers. DS 4.5 years, on prevacid since 5 weeks. Trial wean this summer...fail. Intolerant of dairy and soy DD born Nov 2010. Currently on zegerid. Had stomach malrotation & surgery at 9 months. Avoiding dairy/soy with her while BFing.
Older Kids with reflux
My DS is 3.5 years and has never outgrown his reflux as I was told he would. He's been on prevacid since he was a young baby. Every year we've tried a med wean. He fails them...gets hoarse voice, behavior issues most likely because he is in pain, poor sleep. Our pediatrician has been ok with him remaining on the prevacid. We have seen a GI doctor when he was 15 months. He talked about endoscopy/pH probe study if he didn't grow out of the reflux. Our pediatrician hasn't recommened we do any testing yet and has been fine with him still on meds. He is otherwise totally healthy. At what point do we try med wean again ? Do you gradually wean or decrease down to zantac? In the past we've just dropped the am dose of prevacid for a week or two and then the PM dose. The GI doctor said to go cold turkey but I never wanted to try that. Also, if you have an older child with reflux, at what point did you do an endoscopy? I really want to know if he still has reflux or what is going on. But I also have a baby with reflux and am sleep deprived with her. So I am not wanting to rush into testing/med wean at the moment since I am busy with the baby. But I am figuring by his 4 year pediatrician appointment this July these issues of the meds/reflux will be discussed. Any advice? Thank you!

We stopped trying a total med wean with Jedd a long time ago. We were successful in moving him from Prilosec to Axid though, and there we sit. Any time we miss a few doses because life gets busy, he is a mess.

We have decided that until he can tell us really well what is going on and how he feels, it is not fair to take away his meds. He is just now at 4.5 years old, to tell us how he is feeling. Like, my throat hurts, my belly hurts and such.

As far as an scope, we did one when he was little and was vomitting alot and have much worse symptoms. Have you done one in the past and are looking to do a repeat scope?

In stead of doing a full wean, you can do what we did, which was to move to a lower class of meds as long as it is tolerated. We did both meds for a while, one in the morning and a different one in the evening and then just went for it. It took us about 8 months or so to do the full switch over.

Thanks for advice. No, we haven't done a scope yet. GI doctor wanted to do it at 2 but our pediatrician said we should just wait and see. He was doing so well on meds she didn't think we needed testing yet and thought he would outgrow it. It is frustrating when they can't verbalize how they are feeling. When we did med wean at 2, it increased tantrums and poor behavior so much. I thought it was just the terrible twos at first but once he was back on his meds, it was like a whole new kid. He does get hoarse voices here or there that don't seem to be linked to illness. So I wonder what is going on. But I guess he still can have flares at times. I will ask about a slow wean down to an h2 med at his next well child check up. And take it from there. Thanks!

Most doctors will try this at some point. My son is 8 and still hasn't outgrown it. They have stopped asking now. Seems he will just have it for life. If you don't want to try weaning, and it sounds like you already have, then don't. Our GI used to say to try every 6 months. We never had success going back to zantac either. Maybe you could just try one PPI a day and one zantac dose instead of going all to zantac, that way it the PPI will stay in the system so if you need to go back to it all the way, it will not take so long to kick in.

THe Gi's like to do the scope at some point just to make sure there is no damage from the reflux, or some other issue causing the reflux. My 8yr old has had 3 done in his life already. They are very easy. The make them "drunk" with demerol or something similar first, them do the IV and the sope only takes about 10 minutes while they are asleep. They remember nothing. I'm glad we did it to be sure he didn't have damage, but he still has reflux. If you can afford the scope I would do it. Just remember that if they don't find ANY damage it doesn't mean there is no reflux, just that the PPI has been working.

Thanks for the info. I would like to do a scope at some point to make sure his correct diagnosis is "reflux" and we aren't dealing with something else. And of course to check for damage. If they don't find damage can they tell if he is actually refluxing during the procedure?
