GI appt..not sure what to think
DD (4 months) had an appointment with a pediatric GI doctor at Children's Hospital today. I am not sure what to think about her visit but I am feeling frustrated. She was diagnosed at 4 weeks of age and started on prevacid. My older son (3.5 yrs) has reflux and is still on prevacid since 5 weeks old. Anyway, at 2 months old, she developed a feeding aversion. I've met with OT therapist. Her refusal has been so hard and I've resorted to dream feeding. She is doing better lately. She's been on prevacid 5 mg am/pm and zantac 1.2 mL twice a day. Every time I've tried to wean zantac, she's done poorly. I figured I'd leave both until I spoke with GI. She is pretty gassy and with liquid, exposive green stools. She spits up quite a bit and can be 1.5 hours or so after she ate. She is exclusively BF and I have always eliminated dairy, soy, gluten as those are other family allergies. GI doctor thought she might have some side effects from prevacid and that the zantac alone might work better for her. I have always read and thouht that zantac didn't do much for kids. I told him I was really nervous to back off on prevacid. But now I feel like I have a million ?s on the zantac. Of course, I guess I won't know until I try. Wants me to do once a day prevacid for 2 weeks then stop. Hum...why do I feel so nervous? Maybe cause ALL the trials of prevacid weaning of my older son got us no where. Meant poorer sleep, hoarse voice, miserable times til we got on track. What are your thoughts?

I don't blame you for feeling nervous!! Changing medicine is a hard thing and if you have a (relatively) good thing going it stinks to throw it off!!!

I just started my little guy on Prevacid and it makes him pretty gassy, but so far no explosive stools. Someone did mention to me that it can irritate certain babies since it has a very small amount of milk in it though. Have to wonder if that could be happening for your little one?

I agree that Zantac doesn't have a good reputation as far as fully controlling reflux over the long run, but maybe you can switch and add in Prilosec or something? Would your doctor be receptive to that?

A word of encouragement~ sometimes things have to get worse in order to get better. Hope you find better soon and don't have to go through a lot of experimentation to get there!

Debbie, Mom to:
Derek,10/29/02 GERD,Can't have any legumes(soy, being the worse), allergic to PN/Tree Nuts, struggled to gain weight for years, my "picky" eater,Takes Zegerid. Johnny, 12/05/99 GERD, managed by diet, allergic to cashews/pistachios.
Hi, I agree with you not wanting to go just to zantac. Could you ask to try a different PPI? Everyone reacts different to different meds. My 8yr old had explosive diarrhea every time we had tried Prevacid...and we tried many times because the GI doc didn't believe me, he was BF and of dairy and soy, also. BTW, zantac never was a complete relief for us either, he was on it for 3yrs until I convinced the doc to put him on a ppi. We tried Prevacid, Nexium(both gave him diarrhea). We finally settled with Omeprazole, the generic for Prilosec. Insurance even still covers it as a generic even though they have it over the counter. They also have a liquid if you wanted to try it. You could even do it yourself, by buying the OTC 10mg pill and cutting it in half, crushing it and mixing it with something to give to her. I've resorted to trying things myself many times. Push the doctor. Sounds like you know what is going to work for her already, don't let them tell you different! Good luck!

Thanks for the responses. I did one day off prevacid and DD started to refused to nurse again and had an episode of screaming after nursing for one minute. I chickened out and she is back on her prevacid and zantac. I just can't see her be in that amount of pain again and refusing to eat. I feel like the past couple of weeks she's been doing so well (minus the exposive poops but she's had that for a few months). I feel like the GI doctor didn't fully listen to my concerns. I left there with so many questions. Just didn't have time to process while there and ask more questions. The GI doctor said she is having side effects with the poops and lots of spitting up. He said prevacid can actually cause more reflux ?? but it takes away the pain. I am just totally confused as to what to do. She has a 4 month check up today so I will talk to her pedi about it. She gets shots and rotavirus vaccines today. That set her off at 2 months with refusing to eat. So I am gearing up for a yucky week. Also another reason why I don't want to change things when she gets her vaccines. Ugh! Always a roller coaster figuring things out with these kids.

update...went to DD's 4 month pedi appt. Doctor didn't have note yet from GI appointment (frustrating...I timed them so I would go to GI before the pedi appt). I told her what he said and that I was confused and nervous about his plan. She said since DD has never been on zantac alone and what I am doing is obviously working (she's gaining weight), so just stick with what I am doing and we will disregard what GI said. I am glad pedi is on board with me. I am doing prevacid am and pm and zantac just once a day, mid day right now. I probably could get away with giving mylanta mid day if needed but I can't get ahold of any. Pedi said she has seen explosive poops with the zantac and not with the prevacid. So we will see....I am much more confortable staying on the prevacid for the time being.
