Choking Episode-Need advice
Hi, Today Sadie had a choking episode where she turned purple. Our doctor suggested either continuing with her medications or give her a feeding tube. I wondered if you all could give us any advice about whether tube feeding helped. Sadie is on prilosec, zantac, and erythromycin. She seems to be getting happier in general, but is having more choking episodes (until today she just turned red when they occured). She can always clear her throat, but it is still scary especially today. I think she has a mild food aversion--she'll refuse a bottle in the afternoon, but I can usually make it up through dream feeds. Thank you for any advice! (By the way, prevacid didn't work for us and she's 15 weeks old.)

Jessica, Mommy to Jedd, severe reflux, severe food and oral aversions, Finally G tube free!!! VSD repair 4-24-07 (open heart surgery), hypospadias, repaired 3 times and Ear tubes Visit Jedd at his CB site: www.caringbridge.org/visit/jedd
Have you had a swallow study done to see how effectively or non effectively she is swallowing?? That would be my first step prior to jumping to tube feeds. It could be she needs her formula thickened.

Thanks, Jessica! We're being admitted to the hospital tomorrow for observation and the doctor has ordered one. Any other tests we should request?? She choked about five minutes after eating half an ounce of thickened formula. The doctor said it would be rare for a baby to choke on formula, but I'm scared to death. Mandy

I know you probably aren't too thrilled about being admitted, but that's good news actually.

I'd ask for an Upper GI (UGI) and an EDG (scope) to get done. As well as that MBSS (modified barium swallow study).

Seems like alot but you may as well since you are there KWIM?

FWIW, I know lots of kids online who choke on formula and need thickened feeds in order to make it safe for them to swallow.

If she fails the swallow study, then talk about tube feeds. Even then, she would need to fail it on all thicknesses in order to need tube feeds.

IMHO a feeding tube should be placed if the child is FTT and has been for a long time and is just NOT gaining anything no matter how hard of trying happens, or when medical issues call for it, like Jedd's heart defect, or when a child needs one for aspirating upon swallowing.

You all would fall into the last category IF she fails the MBSS.

What does her weight look like?? Is she falling on the weight/height %tiles?? How much is she taking in in one day as far as formula goes??

Not sure how long you gave the Prevacid to work, can take 2-3 weeks. If she's on enough prilosec, it should kick in. Is she on an elemental formula?
Check with your
doctor first!
