Cyproheptadine (Pariactin)
Hi, everyone, I am new here. This is such a great website! A very brave Mom whose baby has severe GERD and Food allergy recommended this forum to me. I finally feel like home. Well, my son is nine month old now. When he was less than six month old, he was diagnosed having milk allergy due to red blood in his stool. Around the same time, he started refusing the bottle. Two weeks after I was on very strict milk, soy and egg free diet, we stopped seeing blood in the stool. We all thought the allergy was gone, however, his appetite got worse, his food aversion was more severe. Two month after I was on the diet, his condition got so much worse, GI checked his stool with fecal occult card, it was still positive for blood. I was so shocked and couldn’t blame myself enough for being so ignorance (It didn’t occur to me the blood could be invisible). He was still allergic to something in my breastmilk. I put him 100% on neocate (I was pretty sad that I had to stop breastfeeding, but I figured it was the best for him since the allergy had been bothered him for over three months). He developed severe GERD due to food allergy (Silent reflux, again, he generally is a very calm baby, but some nights were really awful, he was in so much pain, I am an idiot for not linking it with GERD). So here we are, more than one month on neocate, no more blood in the stool, but still eats less than 20 oz neocate and 2-3 oz solid food daily. He hasn’t gained any weight for four months. We are currently on both Prevacid (7.5 mgX2) and Zantac. Our GI is very concerned about his poor weight gain. He wants us to start Cyproheptadine (Pariactin) tomorrow to stimulate his appetite. GI really wants to see some weight gain in next two weeks, or else he is going to try to push us for endoscopy. I am desperate to see him eat better, play better. But I am quite scared of putting him on so many medicines. What if his body couldn’t take these much? I feel like I have failed being a Mom, I am so lost that I don’t know what’s best for him any more. Have your little ones had taken these medicines together? Any advice and experience are greatly appreciated.

Welcome!! My 10 month old is on periactin, 15 mg of Prevacid twice a day, and Reglan. The periactin was seriously our miracle drug. Now he still refluxes like crazy but he eats. Seriously it changed our families life! Anyhow, I would go ahead and do a scope. It will tell you A LOT! It will show any damage, food allergies, etc. It makes me sick to give my son so much medicine too but the prevacid and reglan are a must. We have done extensive testing. The periactin I will continue with b/c of his severe oral aversion which can be serious as well. Periactin is just an antihistimine but it can make him drowsy. I give Holden's before his AM nap and bedtime. We could see results after a few days. But for some it doesn't work so if it doesn't then you can pull them off of it. You have to cycle on and off the drug or you will lose the side effect (hunger). I also highly recommend checking into getting a feeding therapist and nutritionist (they can help ypu with calorie boosting) through your state's early intervention program. They are a huge blessing to us.

Thank you so much for your great advice. Now I am so looking forward to trying this medicine tomorrow. We saw four specialists at Mass eye and ear this Tuesday, we gave him a bottle in front of a speech pathologist, wenbo did his usual thing, suck for a few seconds, choked, then pushed the bottle away. The speech pathologist thought Wenbo was still in pain, prevacid alone helped some but not all, that’s why she suggested GI prescribe Zantac for us too, we started Zantac on Tuesday afternoon, it seems to help a little, but he hates the taste. We did schedule appts with feeding team at Boston children's hospital and MGH, I am also waiting to be contacted for early intervention. Wenbo is nine month old, his development is so behind, he can barely sit on his own. It breaks my heart to see him lack of energy most of the time. Thanks again for sharing your experience, I really need it.

I also notice that you are a mom to three, it seems that Holden has been through a lot too, poor little guy. How did you do it, taking care of Holden while care for other two. Taking care of Wenbo alone wears me down, I can't even imagine having two more. I am putting off the plan of having another child.

Periactin
Hi there

My almost 2 year old son with reflux is also on Periactin. He started it a year ago and it was also a miracle drug for us. He now EATS and has put on a HUGE amount of weight. It changed our lives!! We have to cycle it - 5 days on, 2 days off. Sometimes we stop it for a week if the effects are wearing off. Then when he re-starts it, it seems to work again.

He is also on Losec - i think that's the same as Omeprazole - it doesn't seem to be used much in the States but is the drug of choice for reflux here in New Zealand.

Periactin was the only thing that worked for us - i hope it helps you too! Wishing you all the best.

Thank you so much, shan. So I started giving him periactin Saturday before his morning nap, once a day, 1.5 mL. He had 23 oz neocate yesterday, I don't even remember when was the last time he had more than 20 oz, so the first day was great! However, today, he is so sleepy, he pretty much napped all day long. I had to wake him up to give him a bottle. He only took 2.5 oz (last bottle was six hours ago) and a few spoons of solid food (His favorite, avacado and apple). So tomorrow I am going to give him periactin before bed. He was very fussy for a short while in the afternoon, I was wondering if he is taking too many medicines (Prevacid, zantac, miralax, periactin). Well, for your little ones, how long does it take to see appetite increase? how much periactin do you give your little ones daily? Thank you very much!

Zack born 09/28/07 @ 31 weeks. Dx silent reflux @ 6 mo. Now not so silent. Switched to Elecare @ 9 mo. On Prevacid 15mg 1x daily. Looking to stop the puke and gain some lbs.
Puke stopped about 24 months and he's learning to eat...very slowly.
Still on prevacid and periactin.
we were one of the unlucky ones that had no luck on Periactin...it also was compounded with mint...who compounds baby meds wiht mint? Ugh...can't mix that into ANYTHING and Z wouldn't take it well...anyway, didnt work. In case your doc didn't mention it, it needs to be cycled on and off...GOOD LUCK!

My son is on Periactin in tablet form, not the liquid. My Paediatrician tells me he campaigned for the liquid/suspension form to be withdrawn in New Zealand as it is unstable in sunlight - or something along those lines, so it is now available only in tablet form here. My son is on 1 mg twice a day. I crush it and add it to his food. He used to be on a total of 3mg a day when he started it at age 1. I think it takes between 1 and 3 days for it to kick it with him.

You mentioned avocado - i have been told this is a big no-no for reflux kids. I fed it to my son once before i knew this and it made him VERY sick so i have never given it to him again. He was up all night vomitting and crying in pain. Also, my Paediatrician says never to give him green apples, only red. (we also don't give him banana, kiwifruit, pineapple, apple juice or tomato based foods as per our Paediatrician).

Thank you so much for sharing your experience. Shan, you are right, I should be careful with avacado, Wenbo is an allergy baby, our choice for solid food is very limited, he used to take avacado well, but not recently, I am going to stop giving him avacado.

i had never heard about avocado being bad for reflux kids...in fact most Gerd moms give it to their kids to boost weight gain...but I can see how the fat might be too much for them...my son certinaly did terribly with oil in his bottles.
