Back on the rollercoaster
I thought we were off this ride as 10 weeks ago we were able to wean off the Prevacid and for about a month things were wonderful and other then an ear infection and a bout of croup he has been really healthy. So 3 weeks ago Aidan woke up really hoarse and I didn't think much of it because I assumed it was allergies and upped his allergy meds. Well 3 weeks later now he is still hoarse so I finally took him into the ped and he said since there are no other symptoms like a cold or post nasal drip from allergies etc. he has to assume it is reflux again. He has also been choking on just about everything he is eating and sleeping horribly. So he says we have two options just try putting him back on the Prevacid and see if the hoarseness gets better or talk to GI and scope him to see if there is actual inflamation or esphogitis from the acid and go from there. I am torn on what to do. Obviously, it would be really easy to just give him back the Prevacid for a while but part of me wants more answers then that. Part of me wonders why he would still have such severe reflux post Nissen that could cause hoarseness and why he did so well for so many months post Nissen and then suddenly now isn't. Is it wrong to scope him again? He hasn't been scoped post Nissen and it has been over a year since his last scope. He did have an upper GI 5 months ago and his Nissen was fully intact. Any thoughts?

Maybe the reflux has just gotten bad enough to bother him. KWIM I'd just go back on meds. Even doing another scope if they see damage they are just gonna put you on meds. I'd go ahead and do the meds for a month or so and if you see a change for the better than you know if was the meds that were causing the issue. That way you can avoid an OR trip.

I know lots of kids who have a Nissen and still need meds to control the reflux.

With symptoms and a history like that I would want to rescope to rule out EE. My son has all the allergies too & did all the choking like that. The thing is the first scope he had showed only 3 eos (at about 1 year old). It wasn't until the second scope that we found EE (he was close to 2 1/1 years old at that point). At least if you rule that out, you can concentrate on what is actually causing these alarming symptoms. Good Luck, Ellen

Thanks Jess and Ellen. I was thinking the same thing Ellen that maybe it still is EE given his allergy history and that when we first scoped him he was only drinking elemental formula so there wasn't really anything in his system to create eosiniphils. I know it sounds stupid because most likely we are just going to end up putting him back on Prevacid (the GI even said that same thing Jess) but my mommy gut just needs peace of mind knowing that it isn't something else. Thanks ladies for your thoughts.

Yes, scoping while on an elemental diet would not be accurate. As a matter of fact, we are about to go elemental only to get his eos. # way down (where it should be), because his last scope showed 70 eos. After a month or so of elemental only, we will attempt to slowly reintro foods. I hope you get the answers you need to go from here. Ellen

Oh, I forgot to add - they will have your son go pack on Prevacid anyway before scoping him for ee. They want to make sure the reflux is controlled. In that case, up to 10 or 15 eos. in the esophagus could still be severe reflux. Anything above that would be an ee dx. El
