one dose a day ok?
We just switched over to prevacid on Wed. after a bad night and me crying. My hubby called the ped that next day to see if we could get into gi earlier and try another drug. We were on prilosec and it seemed like it was just water for him because his symptoms weren't getting better and then he started getting worse. Anyway, we are now on 7.5 mg of prevacid once a day. I'm having a hard time figuring out when to give it to him because I'm afraid of it wearing off to quickly. We don't want him to be up all night with pain but then I'm worried about him suffering in the morning. I love my ped and am grateful that she works with us but from what I've read babies metabolize the meds 3 times faster so it will wear off faster. He seems uncomfortable starting early early in the morning and then all morning. I give him his prevacid around lunchtime because I'm trying to pick the best time. Any suggestions on what to do. Should I cut the 7.5 in half to give it to him 2 times? I think he's still being underdosed but I hate stepping on my peds toes because she's been so good to us. Our gi appt isn't until April 15 so just trying to make it to that so they can help us further to control is discomfort. Any experiences with prevacid will help. We do make sure it's given on an empty stomach and 15-30 mins before he eats. Thanks

How old is your little guy and how much does he weigh? 7.5 mg is a very low dose. If the pedi would increase it , you should split it. Also you could give him the Prevacid in the AM and try something else like Zantac in the PM. I know you don't want to step on your pedi's toes but if your baby is in pain he may need more med.

Julie - I have three children. My youngest Katie is my refluxer. My first Ben was very colicy and is ADHD. My middle Jakob is pretty easy going. Katie had it bad as a baby no sleep for two years. She was on Nutramagin, Prevacid and Zantac. Thought we were through it but after age three it came back big time. After her scope she was put on Prevacid 45mg a day and mylanta as needed. Scope was in Sept. Showed damage in stomach and espophogus. It took about 7 months of medication and Reflux diet for her to start feeling better.
When Katie was a month old she started on 15mg. If the 7.5mg doesn't seem to help I would be on my peds toes to up the dose to 15 mg. DON'T ever worry about being your peds toes - you have to be even with the best of them.

IF you end up on 15mg, a lot of moms split the dose between AM & PM.

Until it is decided to go up to 15 mg you could ask for Zantac for the PM as suggested above.

I don't know if splitting such a low dose would even be worth it.

I give my 11 week old (9 lbs) 7.5 mg prevacid at 2:30 pm and it seems to work okay. But like I just posted he now has diarrhea from it.

Sara, Mom to Isobel 10/01/08 Dx Silent reflux @ 6 wks. Prevacid 22.5mg split in 2 doses. Pepcid .5ml twice a day. Diagnostically confirmed milk protein, egg and cat allergy. Suspected corn, pea and squash allergy. Trialed Alimentum and Elecare without success, currently on Isomil Advance RTF with moderate success. Barium Swallow and UGI 3/08 Pondering a ph probe and endoscopy.
My GI says that when he weans kids off he leaves the morning dose of prevacid as the last to go. He leaves the night dose of Zantac or pepcid as the last when weaning H2s. Dunno why but HTH

This is our dd, Alessandra. Dx with GER 10/07. Ran the gamut with meds and now done with them (crossed fingers). Using magnesium, probiotics and prebiotic supplements. With the exception of teething and general sassy attitude, all is well now. They really CAN grow out of it!
Many docs will do a PPI and H2 blocker due to the fact metabolism - we did Pepcid in the mornings and then the Prevacid dose in the evenings.

My lo is 3 mos and he weighs about 14 lbs. He was on axid but it did nothing for him so we switched over to prilosec which was a low dose and now prevacid. We just started it and he's just not a happy camper. I hate seeing him in pain and I'm so exhausted. But I know it takes time for the meds to kick in but I can tell when it's working and it does seem to wear off in the wee hours in the morning. I might try to ask my ped if he can be put on zantac for the night time. My daughter was on that and it did very well for her of course that was 9 years ago. I'm not sure that the axid is really going to do it for him for the night. We're just so frustrated that we can't find the right combo of meds. How long did it take a lot of you to find the right combo that worked?

You can ask for a higher dose if you don't' think that one is working. You can also ask about adding in an RX for Carafate. It's like super mylanta. It will coat the throat allowing it to heal underneath and it will also keep the reflux from doing damage.

BTW it took us too long to find the right combo and strength of meds. That's how we ended up 100% tube fed. :-(

Personally, I'd ask for at least a dosing for two times per day. Kids just metabolize them soo fast.

It took us 5 months to get to a stable point. She is on isomil advance ready to feed, takes 7.5mg prevacid morning and evening and .4ccs pepcid morning and evening. Every once in a while she needs a dose of carafate but that's rare these days. This combo has been in effect for three weeks and (knock on wood) it's the best I've ever seen her.

You'll get there too, it's just a matter of trial and error and finding a doc who will work with you. And, the caveat, the magic combo seems to morph and change as they grow so keeping an open mind is important too. I've found that keeping a log of intake, meds and behavior helps me see patterns and keep track of everything. Good luck

My son is 14 pounds at 4 months and he is on 30 mg of Prevacid. That is a big dose ,but I would say 7.5 mg isn't near enough. My pedi says she puts 50 pound kids on that dose (30mg) but according to my GI babies metabolize things 3 times faster than kids so it would make sense a baby would need a higher dose. I would say up the dose to 7.5 mg twice a day. I would try that before adding on yet another med. That is just one more new thing in his system, looking for side effects from that, worrying about the dosage of that as well. The prevacid won't work unless given in the right dosage. Research shows that IF a PPI isn't working that the reason is most likely that it isn't dosed high enough. MARCI kids website has some great info as well as a PPI dosage calculator. That being said I would call my pedi and ask if you could give him one more 7.5mg in the pm. Then when you go to the GI you can say he's on a good dose of PPI it is or isn't working. now what and if 15 mg of Prevacid isn't working he may up it again or he may throw in some zantac. If it was me though , JMHO, I would try a double dose of Prevacid first and wait and see what your GI says. Good luck and keep us posted.

Well you know how reflux always seems like a roller coaster that you can't get off of. Last night was another horrible night and yesterday during the day was horrible too. It seems he felt he didn't need to sleep yesterday so I was exhausted but the night before and day were good for him. I can't explain it because I can't find anything that was different. He slept horrible last night again and today he's having a much better day. He even fell asleep on his own for nap and didn't scream. He seems to be less fussy as well. We still are on the prevacid 7.5mg one time a day and seeing what happens with that before we call the ped. Because I think it might be trying to work itself out with his body and takes more time. I always want immediate relief and it never happens. That would be nice if it did. Well still on the ride and hopefully we will get some sleep in our house soon and find feedings to be more enjoyable. Do you think if I wait until the end of the week that will be enough time to see if the prevacid has shown enough improvement? I know it took a little while with the prilosec but only about 4 days. We're going on our 4th day today. Thanks everyone for all the support and hugs to get through this trial and error I'm going through right now. All the info I've received has helped us get through the really rough spots.

My little guy was on 1 mil of axid and it was not working. Now on the 7.5mg of Prevacid. He did okay the first week then had bad days and good days. Now 3 weeks into it the reflux is not causing him any pain. I would say it took that long before it built up enough in his system. But once again has very runny stools. I would ask your doctor how long they think it should take to start working. Our doctor wanted a check up in 1 month after starting Prevacid to see how it was working.
