Shawna: First time Mom to Mallory (10/07/08). Silent reflux diagnosed at 2 months. Alimentum formula fed. Trial of Losec (minimal effect). Now Prevacid 1x/day plus Zantac 2x/day.
Baby's choking again... now what?
Hi all, just looking for some advice and/or wondering if anyone else has experience with this. Mallory has started choking again and I don't know what to do. It's not a daily occurrence (probably has happened only a dozen times since she was born 4 months ago) but it's very scary for me. Early on it used to happen when she spit up large amounts of formula but the last few times seem to have nothing related to food. She just gets really quiet, then turns beat red, eyes water and you can tell she can't catch her breath. I flip her over, help her catch her breath and then she cries. I thought this was behind us because it hadn't happened since the beginning of January but in the last 5 days it's happened twice. She seems to have a LOT of saliva and is often gulping, swallowing and belching. And sometimes she spits up a couple of hours after eating. Our pediatrician doesn't seem concerned. She initially wanted to do a swallowing assessment but changed her mind because it wasn't a regular occurrence. Has anyone else had this problem? Was anything done or did you get the 'they'll grow out of it' answer? Incidentally, Mallory was hospitalized at 6 days old - she stopped breathing and turned blue (thank god in my arms so I could help her). They said it was a virus but that was never confirmed and I'm pretty convinced it was reflux related.

Has she had ANY GERD related tests yet at all????

I'd def. do a MBSS ( modified Barium Swallow Study) to see what is going on. They may want to do an UGI (uper GI) just to make sure the espophagus looks OK as well.

One of my twins still chokes at home too. She wasn't able to swallow thin/regular texture milk in the NICU and they said she has reflux and wanted a G-tube with fundo for her. She is on Prilosec and elevated bed. To me she doesn't have the reflux symptoms (no spitting up, no crying in pain, no oral aversions yet except when she's not hungry). Her pH probe test was normal. For the most part, she's very hyper (so swallows a lot of air), gassy all the time (pass gas constantly), very playful. She has good weight gain (discharged 3 weeks ago at 9lbs 3oz, now 10lbs 8 oz). But once they thicken her milk with Simply Thick, she's suddenly drinking her entire bottle, that's why she was able to come home without G-tube. Now every couple of days, sometimes twice a day, she would turn blue on me that sometimes lasts almost 20 seconds. She always test out the nipple first and spit out the first mouthful and I think maybe have trouble coordinating between swallowing, breathing, and passing gas at the same time. By 2 months adjusted age, shouldn't the baby figured out how to do the suck, swallow, breathe mechanism? Or is it the thickener that is causing her to choke. She has to use the Y cut nipple for the thick milk. I can't be sure anymore. I didn't update with my pedi or GI doctor that she still chokes because I'm afraid they are going to push for G-tube. My take on it is that even with G tube, I still have to try oral feeding just like what I am doing right now. If it continues, maybe I'll tell my pedi and check her into a feeding clinic. Maybe you should look into that too.

Kathryn, Pager Volunteer and mommy to Emily Mia 02/04/07. 3 years old with GERD. 30mg Prevacid and Pepcid AC. Mylanta needed for flares. Failed prilosec med switch. Tried and failed many med weans but GERD and pain are well controlled. Functional constipation. Reoccurring SBBO. Suspected IBD or autoimmune disease. Resolved issues: MSPI, CSID (acquired), Stage 2 kidney reflux, Chronic diarrhea.

with emily the chocking was a reality for us until she was a little over a year old. We tried reglan b/c of it. when she had a bad reaction to the med and she wasn't a candidate for EES, we were not ever told to "wait it out" but that's what we did- there was no other option. we had also gone through the UGI and endoscopy to rule out structral abnomalities, but that still not not alleviate the fact that she had 2 aspiration pnemonias and bhronchitus. plus the prevacid- anti damage med...

now she is 2 and i honestly never thought i would be sitting here writing this but her chocking days are pretty much over. she still has reflux but she has adjusted to our schedual of small meals and meds.

Emibug, congrats on "living it out". Thanks for the glimmer of hope. Did your baby ever get the G tube? I didn't know Reglan prevents choking.

no she never got a Gtube, the reglan is a motility drug that promots quicker emptying of the stomach and movment through bowles. It was considered that emily had DGE and slowed motility from UGI and SBFT (plus reoccurent episodes of chocking)

that the Gi figured reglan was work to empty the contents of her tummy faster thus less for her to reflux thus less for her to chock... depending on your childs circumstances motility drugs are avaliable and might be something to ask your dr about.

Tracey, mom to Katie diagnosed with GERD at 3 week.
Katie still chokes on liquids and is 16 months. We tried Erythomycin (sp?) since she has a bowel movement every day only on abx. but we had to get a new prescription every 14 days b/c it goes bad quickly. The bowel movement was the only difference we saw so we stopped. Didn't want the side effects of Reglan and since she had no esop. damage we are just waiting it out too.
