what motility test options for 20 mo. old?
My lil girl is 20 months, 19 lbs... FTT--not gaining weight and most recently starting to lose weight! We're considering g tube surgery, but wanted to check out everything... how do we rule out motility issues? Tests at this age? THANKS!

Jessica, Mommy to Jedd, severe reflux, severe food and oral aversions, Finally G tube free!!! VSD repair 4-24-07 (open heart surgery), hypospadias, repaired 3 times and Ear tubes Visit Jedd at his CB site: www.caringbridge.org/visit/jedd
I'd do a GES (Gastric Emptying Scan) and try to get her to drink/eat a normal full meal if possible. Just to warn you, these are not very reliable but are often the first test done.

Are you seeing lots of vomitting??? FTT does not nescesarily point to gastric empting issues. For us, we had copious amounts of vomit well after a meal time.

We did an Antro-Duodinal Manometry test done on Jedd. He basically had probes placed through hisG tube site that went in to his intestines at different lengths. Theses measured the contrations. It was albe to tell how the intestines were contracting, if they did it together of not in sync or not at all. I've find a good link about the A and D man. test for you.

Download the PDF from this site. The author is one of the leading persons for children's motility in the US or the world for that matter. He is the one who did Jedd's test in Columbus Ohio at Nationwide Children's Hospital. And yes we traveled otu of network for those test. We live in VA.

If you get motility studies done I'd go to Nationwide or to Boston Children's. They are the 2 leading centers in the US.

I would also do an Impendance Probe along with these as well. It will give you a more complete picture of exactly what is happening.

let us know if you have more ???

Jessica's advice was right on. I wouldnt do any manometry unless she has already had other testing, it is invasive and can be uncomfortable for the child.Has she had upper GI xray series, endoscopy w/biopsies,modified barium swallow studies, impedance probe, allergy testing???Is she refusing to eat, if so, Is she is in therapy for feeding aversion??Next step is gastric emptying scan...

To add to what Jessica said, motility disorders have many more signs than just FTT.Since your daughter vomits very little(I went through your previous posts) and doesnt seem to have any other issues than some reflux that isnt causing serious complications, I would think it highly unlikely, but not impossible, that she has a serious motility disorder.

All 4 of my children have motility disorders ranging from moderate to more severe. These are the symptoms we see: frequent forceful vomiting, abdominal distension, severe stomach pain(epigastric and intestinal),large amounts of gas, severe reflux that causes complications such as esophageal and stomach damage and respiratory complications such as aspiration pneumonias, asthma symptoms, choking, heartburn, bowel impactions, bacterial overgrowth syndrome, chronic severe constipation requiring cleanouts and daily bowel regimines, dehydration, poor intake and weight gain/FTT,etc..

Kathryn, Pager Volunteer and mommy to Emily Mia 02/04/07. 3 years old with GERD. 30mg Prevacid and Pepcid AC. Mylanta needed for flares. Failed prilosec med switch. Tried and failed many med weans but GERD and pain are well controlled. Functional constipation. Reoccurring SBBO. Suspected IBD or autoimmune disease. Resolved issues: MSPI, CSID (acquired), Stage 2 kidney reflux, Chronic diarrhea.
Audrey Meadow 06/20/08. born 34/35 weeks. Still b/f and eats everything! ftt and chronic constipation.
things like chorns and ulcertive colitus are considers motility disorders and don't often present with vomiting but they do present with other general symptoms such as weight loss and reflux (among other symptoms). Not sure if i would go with a tube or a motility test without first doing a endoscopy or a colonoscopy, but that's just me. if your child has already had these done with little resluts then definitly go with the jessica's advice.

Strong, Beautiful, daughter who is thriving beyond imagination and a healthy, happy 3 year old.
We did the Upper GI series, we have done upper and lower endoscopies, a ph probe, and cannot remember what else at this point in time. The last test we did was Sitz Marker test where my daughter swallowed a lot of little things that looks like minitiare cheerios. Then, we took a series of x-rays to see how long it took them to exit her body and if they stayed together.

I like the sitz marker test because it was quick, easy, and non -invasive. It wasn't scary for her either. Meggy had hers at 20 months and we told her on the way to the hospital for x-rays where we were going and why. She was okay with that.
