need advice reg: 12 mo w/ reflux probls,
My son just turned 1 two weekends ago. He started zantac at 6 weeks, which worked fine for months-- at 7 mos we weaned off the zantac as he started solids. at 9 mos he was a horrible mess-- not eating, screaming through naps & through out the nights-- we got back on zantac which helped a little bit, then finally convinced our dr. to let us try prevacid which worked great for about 6 weeks. for the last month he has slowly gotten much worse. he's very picky about what he eats, and after several bites often quits eating. he still loves oatmeal & pureed pears. it's hit or miss with his eating. he is still on formula (good start soy) but has been unable to switch to soy milk, or other brands of soy formula. he definitely does not tolerate dairy. our GI dr. will not see us again until the end of Jan. He told me over the phone that he does not increase prevacid doses in infants (he takes 15 mgs a day). He had no other advice or help for me. Our primary care dr. does not want to give us add'l meds, or increase prevacid b/c she doesn't want to go against the GI dr. We are gettting very little sleep. Our son screams a minimum of 6 hrs a day. We're all lost & don't know what to do! thank you for any help you can provide. Sincerely, Nikki

My first instinct would be to try Neocate. They have some for kids over 1 (Neocate 1+). If he can't tolerate milk there is a good chance that they soy could also be bothering him. What form of Prevacid is he taking? With the solutabs you have to give them on an empty stomach and wait 30mins to feed. I would also keep a food journal and see if he could be reacting to any foods he is eating. You could look into getting a second opinion from another GI doctor. Has he had any testing like a PH probe?

I was gonna ask about testing as well. Have you had a scope to take alook at things.

JMHO here but if a DR is not willing to work with you aon med doses when a chidl is in that much pain, I'd have to give them their waling papers and take my business elsewhere.

And, yes, I have followed my own advice and it was the BEST thing we hever did for DS.

I would also ask about Carafate for breakthrough pain. that way hopefully you can get him some relief. The GI should be able and willing to do tha tover the phone.

BTW the scope can give you lots of answers. It would let you know what the esophagus looks like and could tell you why he is screamign soo much. It he is in pain the esophagus will show it for sure. It can also tell you if you are dealing with somethign other than GERD like EOS or EE or EG or EC.....those are disorders that can mimic reflux. That could possibly explain why he is such a picky eater.

Also he may be a picky eater because he is associating some foods he has eaten with a lot of pain. That's how our DS was. By the time he was around 16weeks old he was completely tube fed and had refused ALL oral feeding. He stayed that way for almost 18 months.

Don't let anyone tell you that pain will not caus ethem to stop eating, it most certainly can and will if it doens't get under control.

BTW not all kids do that. Some kids will end up comfort feeding to the point that they become overweight from trying to soother the pain.

P.S. the Carafate is by RX o nly. IF the GI won't do it, the Ped should be more than willing to do it.

Julie - I have three children. My youngest Katie is my refluxer. My first Ben was very colicy and is ADHD. My middle Jakob is pretty easy going. Katie had it bad as a baby no sleep for two years. She was on Nutramagin, Prevacid and Zantac. Thought we were through it but after age three it came back big time. After her scope she was put on Prevacid 45mg a day and mylanta as needed. Scope was in Sept. Showed damage in stomach and espophogus. It took about 7 months of medication and Reflux diet for her to start feeling better.
I second the pp. I would definitely find another GI doc. It is not normal for your little one to be in so much pain. A good GI would have upped the meds or change/added. Also, a scope can tell you exactly what is going on.

2 healthy daughters (knock wood) Cory (16), Kim (11) & Eric (4) diagnosed with e.e. (scoped 8/08), GERD & severe food/seasonal allergies, asthma. Cried non-stop first 17 mths. of life Found out he is allergic to cow's milk, soy, eggs, and peanuts. Takes Prevacid, Neocate One Plus & Flovent.
We were in a very similar situation (didn't tolerate diary & screamed/cried way too much. After countless visits to the ped. & GI Dr., I finally went to an allergist to see about a milk allergy. Much to my surprise, he had that & many more food allergies. In retrospect, I can see why. His dad & family have various allergic conditions: asthma, seasonal allergies, eczema. Does any of this run in your families? Either way, all the Prevacid in the world won't fix this, so it's worth ruling out. All the best, I know how difficult it is to have an unhappy baby. Ellen
