New to Forum! Prevacid Effect! Pls help!
One of my twin son, Matthew, 7 mos, GERD baby, has been on Zantac since 2 mos. Recently, his symptoms got worse and the GI said he maxed out on Zantac at 1ml-3 times/day. So we switched to Prevacid 15mg split tab AM/PM last Wed 08/20/08. 5 days have passed and the symptoms got to the worst. He refuses to sleep and has frequent bout of reflux/heartburn before/after eating and during nap/sleep. He used to be a good sleeper and easily fell asleep on my arms while I walked and rocked him. Now it's very hard to comfort him and he falls asleep only when absolutely exhausted. He sleeps less and less and wakes up often screaming in pain. His breath often smells acidic after eating. I know it takes a couple days for Prevacid to work. The GI told me to stop Zantac on Wed. May be I should continue with Zantac for a couple days? Or may be the dosage of Prevacid is too low? I don't know what to do and very confused. How were your experience with Prevacid and switching from Zantac to Prevacid if any? I appreciate any advice and feedback you can give me. I am going to call the GI's office on Mon to ask but I doubt that they can do anything right away other than to tell me wait and see. Thank you very much in advance for your support.

Tried and failed many med weans but GERD and pain are well controlled. Functional constipation. Reoccurring SBBO. Suspected IBD or autoimmune disease. Resolved issues: MSPI, CSID (acquired), Stage 2 kidney reflux, Chronic diarrhea.

yes call your GI asap on monday and ask them if you can continue doing zantac while on prevacid. we were able to do that for my dd it made the transistion much easier, then when he starts feeling better from prevacid we weaned off of zantac. i can tell you from experience going off meds cold turkey makes reflux rebound and hurt a lot! oh also ask if you can use mylanta or maloxx. i hope your son feels better soon, welcome!

Prevacid can take quite a while to work like 2-4 weeks in some children so continue to give it time. I would also suggest some Maloxx in the meantime until the Prevacid kicks in. Since the Zantac wasn't really working it probably won't help much to add it back in. I hope he feels better soon. I can't imagine dealing with twins and GERD. You are one strong Mama! Hang in there and let us know if he gets some relief soon.

Thanks so much for your lightning quick replies. Yes, the Zantac stopped working for us probably 1 week or 2 ago. Looking back now I realized he has been in discomfort for a while but I thought that was because of teething. Then it took us 3 days for the GI's visit and insurance's approval until I finally got the med last Wed. I will try Mylanta or Maloxx. Matthew is usually a very sweet baby and Aidan doesn't have GERD and such an easy baby. Thank God, my mom (a 70 year-old trooper) and my husband help out a lot. Will keep you posted of Matthew's improvement soon, I pray.

Prevacid does take a while to kick in - and excrutiating 3 1/2 weeks for my dd. If you use Mylanta or Maalox for breakthrough pain, make sure you don't give it too close to the Prevacid dose or you'll render the Prevacid useless. Also, just in case you don't know, it should be given on an empty stomach 30 minutes before a meal. I think asking to keep on the Zantac is a very good idea. Hope he feels better soon!

I meant the Prevacid should be given 30 minutes before a meal - when I re-read my post it looked like I meant Mylanta...too long without sleep ;-)

My GI, and even the pharmacist, simply told me to dissolve Prevacid in water or small amount of formula and give it to the baby first to make sure he takes all the med. I did that for the first 2 days. It was by reading messages on this forum that I found Prevacid should be given on an empty stomach 30min before meal and have been doing that for the last 3 days. Will ask the doctor on continuing with Zantac. Thanks a lot for all of your advice. I feel stronger and more optimistic to continue with Prevacid.

Melissa - Mommy to Sebastian, born 9/28/07 @ 7 lbs 7 oz and 20.75 in. At docs at 12 months: 27 pounds and 32 in! Good grief! GERD - currently on Prevacid suspension (liquid equivalent of 45 mg a day). Allergic to dairy, soy, eggs, wheat, all treenuts, peanuts, fish, shellfish, peas, sesame, bananas, barley, coconut, dust, dust mite, dog, cat, latex. Seasonal allergies. Oral Allergy Syndrome (OAS). Neocate only - no solids - until further notice (drinks and loves water, too). Loves his doggie, our rescued Treeing Walker Coonhound, Louise, even though he is allergic to her. Louise will make your doctor a homemade pie if you would like her to. PHOTO: We couldn't do a cake on Sebastian's birthday, but that didn't stop him from tossing around the contents of a big bowl of popcorn! He now tries to eat it when we play with popcorn, so no more of that game.... :(
It took us a good month or so to get results with Prevacid...and I wish we had stayed on Zantac or something while we waited for it to kick in. I didn't even know that was an option...

We never had success on Zantac (or Prilosec, for that matter), and most kids have the same story on this forum. Give the Prevacid a little time to work. The acidic breath should ease up a bit as the PRevacid builds up in his system, but Sebastian still gets bad breath all the time. I hope your little man feels better soon.

Our 1 year old daughter has silent reflux, hypotonia as well as benign external hydrocephalus. We are currently weaning onto whole milk (we'll see if it works) and she is also on Prevacid 15mg per day.
We had to do the Zantac while starting the Prevacid for quite some time. It took almost 3-4 weeks before I felt that the Prevacid was working. Our daughter is on 15 mg of Prevacid per day. She's been on that dosage for a long time now. I hope the doctor will let you use both for awhile. HUGS!!

Jennifer - 1st time mom. Lauren (6/1/07) diagnosed 9/07 with GER at 3 1/2 months. Prevacid 7.5 mg in the am. Currently drinking whole milk and eating great!!
No longer aspirating! Yippee!
Picture: Why would Lauren play with the hundreds of dollars worth of toys scattered in my living room?!?! When she could play in a box! Estimated move date: Oct. 3rd!
We got much better success from Pepcid rather than Zantac. We are currently doing a Pepcid/Prevacid combo. They are different types of medications so they work differently. It did take what felt like forever for the prevacid to start helping, but hopefully it will kick in soon for you. We did Mylanta a lot at the beginning too, for a quick fix, but beware, too much mylanta can cause a blowout diaper. I hope your ds gets some relief soon! Welcome to the forum, sorry you have to be here, but know that I have learned more here than in ANY doctors appt!

This is our dd, Alessandra. Dx with GER 10/07. Ran the gamut with meds and now done with them (crossed fingers). Using magnesium, probiotics and prebiotic supplements. With the exception of teething and general sassy attitude, all is well now. They really CAN grow out of it!
Welcome! We also do the pepcid/prevacid. We do pepcid in the mornings then prevacid in the evenings (easier to give on the empty stomach then - not as desperate for food!) My dd's MD has always had her on an H2 blocker along with a PPI, since little ones tend to metabolize stuff so much faster. It took about 6-8 weeks for us to get the full results of prevacid, though improvements came throughout the wait.

The empty stomach bit - both our MD's and the pharmacist have all told me the pepcid or any other H2 blocker can be given anytime, food or not. The prevacid should be givein at least 1 hour, preferable 2 after the last meal and at LEAST 15 min before the next one, and NO OTHER MEDS within 2 hours (before and after) of the prevacid. Once we got that all straight, things really did start improving! Hang in there - we're thinking of you and everyone here has fantastic advice and support!

Cooper was born 12/19/07. Birth weight: 7lbs10oz. Weight as of 2/11: 8lbs9oz. 6/23: 14lbs12oz! Struggling again 12mo: 18lbs. Breast Fed exclusively, on strict elimination diet until Dec 08. MSPI as well as other food intolerances Diagnosed w/ GER at 5 days following barium swallow. Choking with apnea spells. Ftt. dx w/ Laryngomalacia following brochoscopy 1/29. Aspiration. apnea monitor until 5mo. pH probe 1/29 showed severe reflux. Sandifers Syndrome. lbrady and tackycardia. echo and EKG 2/18: Came back normal! Swallow study 2/29: some NG reflux. Unexplained seizure 4/1. Negative CF sweat test 4/4! EGD 11/19: reflux damage in esophagus. pH probe 11/19, reflux improved. Safe foods: pears, apples, peas, rice, small amounts of chicken, potatoes, and carrots.
Currently drinking Elecare vanilla 30cal/oz and breastmilk.
Meds: Nexium 10mg 2x daily. Carafate as needed, miralaxx as needed, probiotics daily
Welcome. So sorry you are having such a time. I agree w/ pps but I wanted to add Cooper did not do well w/ the tabs. We always had a much better time w/ the capsule poured on a bit of baby food. you might ask about that.

Coyle Eagan. One Year Old. Doing well on Cow's Milk flavored with juice :) Drinking two bottles of Go&Grow daily. Eating solids better but still have rough days. No More Silent Reflux-that I can hear anyway.. but puking again. Zantac 2ML 2X daily.
hi and welcome. how much does your son weigh? mine is only 16 pounds and takes 4 ML daily 2ML twice a day. this helps but of course doesnt take away all the pain. we've tried other meds (prevacid/prilosec) with no success...

id ask about upping the zantac or possibly combining with prevacid until it starts working. also, i've been told by several doctors that even PPIs can be given the same time as food... but we still always tried for 30 min prior. we just put the tab right in his cheek and let it dissolve.

Tracey, mom to Katie. Diagnosed with reflux at 3 weeks and put on Zantac. At 8 months old she was removed from Zantac as it was giving her migraines, currently on Prevacid. Switched to Elecare since she is not gaining weight; trialing solids after being off for a month. Geneticist said reason she isn't gaining weight is she is stubborn. Upcoming appts.: Neurologist for developmental delay and EGD scheduled for September. She is one funny baby!
Hi there! I only have a moment to post; Katie was maxed out of Zantac at 7 mos of 1.5 mls. 3 x's a day and it was giving her migraines so we switched to Prevacid. Prevacid took over 6 weeks to really start to work. She stopped sleeping, eating, went from constantly whining to constantly screaming, throwing up what she did eat and never able to sit still. Once the Prevacid started working she was a different baby altogether; she started playing, sleeping without screaming while asleep, actually wanting to eat and so on. The mylanta did help a ton but our GI didn't want her on it so we had to give it to her against medical advice (not advocating but we had to do it). Prevacid really helped but it took a lot longer for us than some other GERD babies.

Thanks a lot for everyone's advice. I was able to talk to the GI's nurse only today. (Ped GI doctors in our area are very hard to catch, appointments need at least a couple weeks or months to make). She told me NOT to use Zantac while on Prevacid. But I remember the GI told me at the last appt that I could use Zantac alternately with Prevacid. However, he also said that I could stop Zantac last Wed afternoon and the Prevacid effect should kick in in time. Anyway, I decided to add back Zantac for now until Matthew feels a bit better. He could eat more today but still sleeps poorly and wakes up screaming. He fights ferociously at nap/bedtime. I almost dropped him since he pushed strongly and squirmed away. My husband took over. He has stronger arms to hold Matthew. I'm so concerned with Matthew's serious sleep deprive. I wonder why he fights sleep so much even though he's exhausted. May be he's in much pain during sleep and then associates sleeping with being in pain? Or could be Prevacid's side effect ??? Did you have the same problem with refusing sleep? The nurse also said I could use Milk of Magnesium for his constipation, (he still poops once a day, but the stool is very hard and dry), and Mylanta is ok to use. My son's weight is 17.5 Lbs and the GI said he maxed out on Zantac at 3 ML (1ML 3 times/day). I asked the nurse today about med combo (Zantac or Pepcid with Prevacid) but she told me flat out that "they don't do med combo" and that the "2 medicines are working the same way" :((... Well, for now we just continue with Prevacid until it works. Matthew gave us big heart melting smiles today which meant a world to us.

interesting. different doctors will tell you different things. do you go to a children's hospital? the 2 medicinces do not do the same thing. zantac-an h2blocker neutralizes the acid in the stomach. prevacid is a PPI-it stops the proton pump from producing acid at all. many moms on here have been advised that in the short term they are fine to use together. can you talk to another doc in the same pracice? some are just better than others. i did not like our first GI so i ended up scheduling with the director of the department instead. he is hard to get in to see but his nurse is wonderful.

as far as the sleeping. i noticed around that age that coyle would push away from me at naptimes-on occasion. he was just asserting independence. i would lay him down in his crib and then go back in in 5 min. if he had not fallen asleep yet. he would then let me rock him for a bit.

many kids wake up screaming because they are sleep deprived. they are normal and many go back to sleep on their own or some have to be goaded back to sleep. they do this during the transition between light/deep sleep which happens several times a night.

oh, and many kids will fight sleep just because they want to stay up and "hang" with us even tho they are so, so tired! my son will shake his head vigirously in an effort to stay awake.

what is your nap/bedtime routine?

I feel like a guinea pig and frustrated many times after seeing doctors since everyone says different things. I don't think the GI's nurse really knows what she was talking about when she said that Zantac and Prevacid are working the same way. My last pediatrician refused to up Matthew's Zantac dosage from 2ML (1ML twice a day) and even talked about weaning him off at 4 mos. I changed pediatrician because of that. Now I seriously consider switching to another GI in the same practice (the 1 and only ped GI practice in my area) even though it takes months to get in his/her schedule. Before this reflux rebound, Matthew used to nap around 9:00 and 1:00 PM for 1 or 1.5 hrs, then sleep around 7:30PM, had his last bottle around 10:30PM and then sleep until 7:00AM. I used to hold and rock him until he feels asleep (usually it took only 10~20min) and wait until 1.5 hrs after feeding to put him down to the crib. He rolls over to his tummy as soon as I put him down, sucks his thumb a couple min and goes back to sleep. If I put him down too early, he rolls over, sucks his thumb, cries to sooth himself and sometimes throws up so I'm scared. Now it's very hard to soothe him. Eventhough, he is exhausted, yawns 5 - 10 times, even falls asleep in my arms while I walk him downstair, as soon as I bring him to his room and close the door, he shrieks, gets alert, and cries more and very cranky. I have to bring him downstair, keep up with him until he gets more exhausted, then bring him up and start the soothing process again. Now his nap/sleep schedule is out of control. He naps 30min or 45min if we are lucky, crash to sleep somewhere between 6:00PM ~ 8:30PM depending on the day. After his last bottle at 10:30PM, it takes 1~2 hrs for him to go back to a deep sleep state, and then wakes up several times between 2:00AM to 6:00AM. Sometimes he can goes back to sleep on his own, sometimes he needs soothing and formula eventhough he has heartburn after feeding and takes 1~1.5 hrs to go back to sleep. He is in a sleep deprived state, hardly smiles and NOT like himself at all. I am terribly concerned if it's the Prevacid side effect. The nurse told me it was not though... I really pray that once the med kicks in he feels better and can sleep more.

yeah, many of us feel this same way. our pedi thinks that coy's weight gain is fine-the GI says no. one nurse tells you one thing-another tells you something different. the best thing to do is talk to other people in your area that you know and trust. where do u live? maybe someone on here can give you an idea of who do see. that is actually a pretty great schedule. i don't want to downgrade any pain he may be having, but the issues you are having sounds more like the start of seperation anxiety/independence. that is exactly how my son behaved (who has reflux) and my GFs son (who does not have reflux) at that age-7 mo. and still does on occasion. liek today he completely refused an afternoon nap; fell asleep from 4:15 to 5:00 and then refused to have dad put him to bed @ 8-even tho daddy was fine last night. many of us on here do not advocate "crying it out" because of our kiddos reflux. so, we've all had to adapt to giving them the independence they need to fall asleep on their own without causing unnecessary pain. what works for one does not work for another.

my suggestion would be 1. find a doc willing to give your son the right combo of meds. my little guy does great on 4 ML of zantac daily and he weighs less than your kiddo. 2. check out www.sleeplady.com and see about getting her book from the library. she has a whole section on reflux babies. also look into weisbluths' sleep book too...

Thanks mom2coy. Your son is very cute. Yes, I think Matthew's sleep problem could be partly related to separation anxiety. Both of my twins are going thru that phase now. I'll keep an eye on that. I live in Orange County, Southern California. If anyone knows great a pedi GI, please introduce me. Thanks in advance. I'm a big fan of Dr. Weissbluth. I will check out the Sleep Lady's.

yea, and this phase is esp. harder on your son with reflux because the crying aggravates it. i think there are a lot of moms on here in california. id post a seperate post asking for some suggestions.
